Disability Care - NDIS

Your Aged Care at Home is committed to supporting you live at home and do more of what you enjoy. As a registered provider of the National Disability Insurance Scheme we offer a large range of support services that will assist you with your day to day activities.

Your Aged Care at Home is also able to assist you in understanding the NDIS or coordinating your plan, taking out the hard work involved in getting the quality of life you deserve.

What is the NDIS?

The NDIS helps provide individualised support for people with disability; including their families and carers. The NDIS provides all Australians with a permanent and significant disability, aged under 65, with the necessary supports they need to live an ordinary life.

Accessing the NDIS

People who meet the NDIS access requirements are called participants. Participants receive funded supports and an individual plan.

If you are a person with disability and want to join the NDIS, you need to meet the access requirements. These include: availability of the Scheme where you live; your age; your residency status; your disability and in some cases your early intervention requirements.

Personal Services

Bathing, showering, grooming, dressing. Toileting, mobility transfers.

Activities of Daily Living

Assistance with impaired hearing, sight or speech.

Nutrition and Diet

Meal preparation and assistance with eating. Providing enteral feeding formula and equipment.

Core Supports

Assistance with Daily Living (1.01)

  • Assistance with self care (regular and high care)

  • Assistance with personal domestic activities (regular and high care)

  • House and/or yard maintenance

  • Linen service

  • Assistance with preparation and delivery of meals


Transport Support (1.02) 

  • Assistance in travel to access the community for educational, recreational and vocational activities.


Assistance with Social and Community Participation (1.04)

  • Assistance to access community, social, recreational activities (regular and high care)

  • Group based community, social and recreational activities

Capacity Building

Continence Management

Assessment for/providing pads, commodes, bedpans, catheter and urinary drainage appliances. Assistance with continence aids and appliances and managing continence.

Management of skin integrity

Providing bandages, dressings and skin emollients.

Domestic Services

Mopping, vacuuming, washing dishes, cleaning bathrooms, making beds, dusting, and much more.

Laundry Services

General laundering of clothes and linen. Arranging dry cleaning services.
